Where can I find boilerplate language for a Homekeeper disclaimer?

Hi Liz,
We are looking for some boiler plate language that let's applicants know that if they provide us all their information, it will be aggregated by Cornerstone for the purposes of statistical analysis and no personal identifiers will be utilized. Do you have any language you can share? We are still a bit always off from having our data in a condition that you can do a social impact report....but that is our long term goal.

Here's a link with some information/language: https://homekeeper.zendesk.com/entries/20693023-What-types-of-data-will-be-uploaded-into-the-sector-performance-data- There is also language in the member agreement about how we use the national roll up data. Because state laws vary so much, I know we haven't been able to offer a "HomeKeeper" disclosure.
